Marine Sports Participation Agreement and Consent Form

If you wish to participate in marine activities, please be sure to read this page to the end. If you agree with each item, please ✔ mark □. Please understand that if you do not agree to the terms and conditions, you may not be allowed to participate in the activities.

I am aware of the risks associated with marine sports and agree that I am personally responsible for all risks, and injuries, foreseeable or otherwise, resulting from such activities.

I wish to participate in jet skiing, banana boating, screamers, and wakeboarding with full understanding of the potential dangers involved.

I agree that I am solely responsible for any and all disability, other damages, accidents, theft, or loss that may occur to me and agree to hold Guam Ocean Park and Sunny Side Up harmless from any and all consequences.

Medical history and questions

I confirm that the following six items are accurate and that I will respect the instructor's guidance and instructions and participate of my own volition (if any of these items apply to your medical history, please consult your supervisor).

Currently pregnant.
Have asthma or experience wheezing when exercising.
Has ever fainted or blacked out.
Has impaired motor function.
Has frequent dislocations or sprains.
Has a history of head, back, or hip injury.

No extended fingernails on hands or feet. Or, my fingernails and toenails are extended, but I participate at my personal risk.

Naked eyes. Or I wear contact lenses, but participate at my own personal risk.

I have not been drinking alcohol. I understand that if I am under the influence of alcohol or have been drinking, I will not be allowed to participate in the Activity.
